The mesh should be a single connected character rather than a scene, and a neutral A-pose or T-pose with the limbs separated from the body gives the solver the clearest read. Sculpts out of ZBrush and retopologized meshes from Blender or Maya both work. If the arms are crossed or the legs are fused, straighten the pose first and the skinning improves noticeably.
